DentaQuest processes claims for 2 payers with a claim mailing address of 12121 N. Corporate Parkway, Mequon, WI 53092 - MA Medicaid and Doral. Each payer has a separate payer id number which must be submitted within the claims.  Failure to send the claims with the proper payer ID number will cause those claims to reject at the payer.  If you are unable to send the proper payer ID number you may submit the universal payer ID of 06126 and the proper payer name and mailing address as displayed on the patients insured ID card.

 

Sending claims for a MA Medicaid patient with the CX014 payer ID will cause those claims to reject at the payer.  Conversely sending claims for a Doral patients with the CKMA1 payer ID will cause those claims to reject at the payer.
